Description

It is critical for agencies to understand the importance of each of these conditions, and the preparation time that will be required in order to be compliant by January 13, 2018! This session will review each of these conditions and give agencies information on how to comply, from policies and procedures, to staff training, EMR or paper document revisions, and qualification changes. Patient Rights will review all of the timepoints needed for each of the areas, such as providing verbal and written notice of rights to patients and patients' representatives. They will also review what written notices should be provided to patients, and the involvement of the patient and patient representative throughout the home care admission. QAPI will review the standards, and give strategies, tips and examples of how to develop and implement your program for compliance and to help your agency achieve great quality and patient outcomes. Infection control condition is sparse, but Sharon will provide you with information on how to develop your policies, complete an infection surveillance form, and perform staff and patient teaching. Finally, Personnel will review the new qualification requirements for administrators and clinical managers. This is a critical session you don’t want to miss as you prepare for our new CoPs!

a. Patient centered holistic care
b. Interdisciplinary team working with patient and physician to improve patient outcomes

2)Patient Rights Condition
a. Each standard will be reviewed to include the Final Rule comments in federal register
b. What the agency needs to implement in order to be compliant
c. The timeframes and descriptions in which notifications must be given to the patient and pt selected or legal representative and/or caregiver(s)
d. Policies that will need to be changed in order to be compliant
e. Training that needs to be given to staff

3)QAPI Condition
a. Details of each standard
b. What is required of the agency by jan 13, 2018 and not until jan 13, 2019
c. How to develop and implement a successful, compliant QAPI program

4) Infection Control and Prevention Condition
a. Details of each standard
b. What is required of the agency by jan 13, 2018
c. How to develop and implement a successful, compliant infection control and prevention program

5)Personnel Qualifications Condition
a. Details of each standard
b. Administrator changes in qualifications
i. Requiring an undergraduate degree for all new administrators of an agency
ii. Grandfathering in current Administrators while they remain at the same agency
c. Clinical Manager description and standard
d. Allowing therapists and MSW to also be clinical manager
