40 years ago, psychiatrist Erik Erikson described each patient as “a universe of one.” This concept is no less prevalent today, and in fact has taken center stage. Our world is changing and within it, the perception of managing your own healthcare is changing as well. The fact is, the information before we even get ill will contribute to a healthcare system that is more self-aware and more efficient. In this session, we will explore the emerging landscape of patient engagement. We will delve into the data gathered and ACO drivers -- all developed in efforts to support the Triple Aim: population health, experience of care and how we can lower costs of care.
