Description
The question home care providers often ask themselves is what they should focus on to improve patient — and therefore agency — outcomes. This session will focus on key outcomes as well as assessments, the Outcome and Assessment Information Set (OASIS), and care coordination. Several action items will be provided with strategies, such as education for staff and performance improvement initiatives based on Quality Assurance and Performance Improvement (QAPI) indicators.

Objectives
Course Objectives:
- Understand how to select key problematic outcomes to focus on improving.
- Be able to develop a realistic action plan to improve low outcomes.
- Develop an education plan that will engage staff so they understand areas for improvement and can implement each.
- Provide care with an interdisciplinary team working together in order to coordinate the patient care in order to improve outcomes.
- Develop Performance improvement projects for complex outcomes that may include several various outcomes and multiple staff and processes.
- Develop quality indicators for low outcomes in order to improve and then sustain the outcomes.
