The purpose of this session is to "show you the money" under the Expanded Home Health Value-Based Purchasing (HHVBP) Model. Attendees will learn how to improve patient outcomes by re-engineering the discharge process in a progressive and innovative manner.
Objectives
The following are some examples of learning objectives to be covered:
- Attendees will understand differences in Condition of Participation Requirements for Discharges and those required for OASIS E.
- The learner will identify at least 2 innovative solutions for decreasing hospitalizations post discharge.
- The learner will identify at least 2 requirements for Reconciliation of Medications.
