This session will discuss social determinants of health, health literacy, and the challenges associated with measuring them. As part of the presentation, these concepts will be linked to the Outcome and Assessment Information Set (OASIS) for home health agencies.

  • Learner will identify factors that impact measurement of HL to understand it’s complexity.
  • Learner will describe the Social Determinants of Health.
  • Learner will relate HL and SDoH to barriers to care and OASIS-E accuracy.
