Description

Home Health programming has changed under PDGM, and modifications to how we develop and deliver rehab services have been a primary reform focus. When transitioning from the PPS model, with rehab visits as the primary payment factor, to PDGM programs based on clinical acuity, therapy care plans have been all over the map. By trading rehab volume for content value, PDGM has altered the therapy care landscape for all Home Health episodes, and in many cases, maintenance programs have been lost in the shuffle. In this presentation, we will discuss how Maintenance episodes can be optimized under PDGM, and how they differ from PPS era programs. Under PDGM, rehab programming should follow structural guidelines, including SOC FIL levels, and should be managed through an in-agency approach with ongoing rehab reporting. We will review these and other essential elements of Maintenance Therapy development and delivery under PDGM in this progressive presentation.
