Consider this scenario in your agency: you have a visit nurse in your organization that is doing an excellent job. Patients, staff and physicians love and respect this nurse. The current DON gives notice, so who you gonna call? The logical progression of promotion in the home health industry is from staff nurse to Director of Nursing, usually with minimal training. How soon can this be done, who will assume the case load of the visit nurse if you make this change? Time passes, no replacements are forthcoming. This is that OMG moment when your good judgment abandons you and you decide to move this nurse into the office of the DON and promise on the job training. About 75% of the time this is how this happens. Is this a fair situation for anyone? When you review the duties and responsibilities of the Director of Nursing as established by the Florida Administration for Health Care’s minimum standards for home health agencies and then you review the Federal COPS requirement for a “Supervising Nurse” and the accreditation standards for this position, you begin to realize this might not have been the best decision, but by the time you, as the manager and the new DON realize this, the “new” DON is pulling her hair out. We have essentially set her up for failure. Well there is good news and we have it.

Initially we will provide you with current rules and regulations defining this position and then on to exciting ideas for creating a less difficult transition from visit nurse to an effective Director of Nursing. This session will include discussions of Florida home care organizations requiring a DON designation; Chapter 400, Part III, Florida F.S. 2012; Chapter 408, Part II, Florida Statues and Florida Administrative Code, Chapter 59A-8.
