Through the years the Health Care Industry has seen expansion of multiple payment models, payment reform, and changes related to rules and regulations.  All revolve around a common subject – “the patient”.  This presentation will discuss wound care resolutions  within the home care scope of practice including best practices, assessment tools, and product efficacy that will embrace optimizing patient outcomes in a cost-effective manner.

Objectives

At the end of the presentation the participants will be able to:

1. The participant will understand the foundation of wound assessment including identifying the depth of tissue layer destruction and understand why wounds differ in the method of the healing cascade.
2. The participant will understand characteristics that will assist the clinician’s communication with the physician to determine accurate etiology of wound type to assure adequate documentation and coding.
3. The participant will develop a basic understanding of available treatment options and methods mitigating lack of wound healing progress.
4. The participant will understand Best Practices as it relates to quality domain reporting for skin integrity since the January 2017 implementation of the “Impact Act of 2014” for the homecare setting.
5. The participant will understand what Fee For Service, Accountable Care Organizations, and Managed Care have in common when it comes to patients and wound care.
6. Participants will understand The 2017 Home Health Conditions of Participation as it correlates to wound care.
